Parent and Provider Contract We understand and agree to the following: Parents are required to pay tuition at Monday drop-off. Pay in advance. If a check is returned due to insufficient funds, parents will be responsible for ALL associated costs
including bank charges. In cases of non-payment parents will be responsible for ALL associated costs, including small claims
court and lost wages. You must pay for the days and or hours, your child
is booked in for daycare, even if they are absent! Fulltime children have to pay for 4 days per week, to hold your childcare
spot. Partime children have to pay for the days they are booked in, even if they are absent, to hold their spot Fees: The rate for childcare is $_________________ per child. The rate for childcare is $_______________per 2 children or $_________per hour for 1 or $________per hour for 2 The rate for N.S.F. is $50.00 per checkThe rate for late pymt. is $10.00 per day for every day that payment is late. holidays = double rate! Overtime: Overtime is considered any time outside of the agreed hours of care. I will allow 1 hour
travel time for parents working out of town, Or more if needed by my discretion. Payments: payments are to be paid every Monday at drop-off for the week in advance! If this day is
a holiday, payment is expected the next day. (Tuesday) Illness Policy: If your child is booked in for daycare and you cancel for any reason, you still have
to pay for that day. If I am sick and close the daycare, you will not have to pay. Late Arrival and Pick-up Policy: Please advise me if you are going to be early or late picking up or
dropping off. Late fees may apply. Holiday Notice: I have to have at least a months notice of holidays
scheduled. If I don’t receive a months notice you are required to pay for your daycare spot. All parents are allowed
2 full weeks of holidays per year. After you have used your 2 weeks holidays, you are required to pay for the daycare spot.
(2 weeks free holidays applies after 1 year of placement) I keep track of everyone’s holidays in a book, in case you
forget. The daycare is closed on the following days: Christmas Day, Boxing Day, Easter Sunday, Thanksgiving, July 1. Other
days will be posted at least a month in advance. IN_____ This contract may be terminated by either parents or Provider by giving a month written notice in advance
of ending. If your child is withdrawn without the required notice, a months fees will be due upon termination. This contract is effective________________________and subject to
renewal and change with a two weeks notice. IN______ I/We have read, and understand and agree to comply with the commitments and policies stated in this
contract. I/We understand that this is a legal and binding contract between Leslie Tiller and my family. I/WE have been given a copy of Leslie’s policy and this contract
for future reference. I have received a copy of Leslie’s Handbook and I have read and received a copy for future reference.
